I believe you can also get this on modification of non-java files, not just
creation, if the file is under the source folder.

We have identified and resolved a bug in the Java builder which incorrectly
marks as dericed new non-java resources added inside source folders
(instead of their corresponding copies inside the output folder). This
results in these original resources to be ignored by the VCM resource
management.
A patch for this issue is available at:
http://dev.eclipse.org/viewcvs/index.cgi/%7Echeckout%7E/jdt-core-home/org.eclipse.jdt.core241b.zip
It can be applied to a 20020416 (M5) build or better (by expanding it into
the plugins folder).
