I think it's important to refer to the Eclipse committer guidelines for
this (http://www.eclipse.org/legal/committerguidelines.php).

For instance, it is the committer's responsibility to make sure the code
being submitted is submitted under the EPL.  The best way to ensure this is
to make sure the changes are committed through an attachment to a bugzilla
entry.  For large contributions, the submitter must also complete a
contribution questionnaire
