it seems that we don't have any problem with the 
26591 projection. I
really don't know what you mean with it doesn't 
work but attach here
some consideration, hope they would be useful.
1. Monte Mario in both the EPSG codes 3003 and 
26591 works well just if
you work only with other Monte Mario projections
2. if you need to put together things in Monte 
Mario and UTM so you have
to modify the prj files adding the TOWGS84 for 
both projections, if you
don't do that your features will be shifted (I 
tried to attach here a picture but the mailing 
list doesn't let me send it... if you need I can 
send it directly to you)
3. if you change the prj file of your shapefile 
and substitute 26591
with 3003, even if it is wrong... nothing 
changes!! so I really don't
know what happens...
