I don't have a SQL manual handy, and I'm up against a deadline,
so I don't have time to look it up.
I don't see anything in principle wrong with generating indices
manually. On the other hand, this is *exactly* the kind of thing
that the new database abstraction layer was intended to handle. I'd
kind of like to see two different implementations, one of which
generates the indices manually, and one which lets it be done
automatically - just because I'd like to see a significantly different
implementation.
