I'm working on a bunch of bugs that are interrelated, involving the command service, layout of commands in the UI, DOM structure cleanup in the nav, etc. [1]
For the next day or so, I'll be releasing some piecemeal changes to avoid a monster set of changes and a possible painful merge.

You will notice some of the commands moving around a bit, but their placement does not necessarily reflect the final solution.

For example, the nav item commands have moved to be closer to the item name. This is intentional, but there will still be some new commands coming and some reorg into menus.
The breadcrumb actions have moved to an unadorned row underneath the breadcrumbs. This is an interim step toward a more structured presentation, and it will get additional polish soon.
