That Implementation guide does need to be updated to the latest

It was initially developed outside of the MDHT project and has not been completely committed to the project

There are several other model projects that need to be updated as well

It seems there's a bug in the QuestionnaireResponseDocumentHeaderOperations as there are multiple string constants that ought to reference a line in the plugin.properties file. For instance, the string 'QuestionnaireResponseDocumentHeaderQuestionnaireResponseDocument' is nowhere to be found, and it looks to me that the part 'QuestionnaireResponseDocumentHeader' has erroneously been repeated twice.
