the publication starts with your spec-book.ditamap which typical includes the use of the content.ditamap
You are free to modify the structure (currently manually but you automate) to use your package structure

I am not sure of your second question - Are the packages not being processed ? 

In general - you should structure your model in which ever mode you would like - with the caveat that we currentl do use that as part of the API
The DITA is standard is designed to mix and match you content; we do generate a default layout but we customized for consolidated CDA

One last note -  Are you looking to have these used as HL7 Implementation Guide ?  If so, they have their set of rules to publishing

What I’m trying to achieve, among others, is to group each segment template with related clinical statements in one section (i.e., chapter) of the generated PDF.
 
Another related question is how can I build a “packaged structure” (see the attached picture). If I create such structure from scratch it is not translated to DITA, if I create a flat structure first and then repackage all templates/classes they appear in PDF as usual but new packages or classes will not.
