Hi David - I assume you are working with the NEHTA extensions -you need to apply the EReference stereotype but this is not straightforward in the table editor as you select the association and not the property type
If you use a uml editor you can set it ( but you might run into issues with the path maps of the base models)

Which one is missing an extension as i had thought i had set most of them

I assume you are asking about the XML namespace, not the UML namespace that contains the Association?  What is your requirement for using a different XML namespace for a UML Association?  The UML Associations are not serialized to CDA xml documents, only the origin end Property of a navigable Association as an XML element.
