Yes, there have been some changes in LSP4E and transitive dependencies between 2018-12 and 2019-03.

I don't know what specifically can cause your issue. I think you'll need to debug LSP4E and find out by yourself. Best thing is that you open a bug to track progress.

LSP4E doesn't really contain automated test handling workspace folders. There are some stuff about dynamic registration but no such test about the flow of opening a new folder, sending the notification... It really seems like an important missing piece for which a contribution would be very welcome.
