Bug 342632 - Duplicate CascadeLockingPolicy instances added to private owned child

Fix to ClassDescriptor, adding a new internal flag: cascadedLockingInitialized, and initialization checks to ensure that duplicate cascadeLockingPolicy instances are not added when multiple descriptors in a model are initialized.

Tests: 1 new (CascadedVersionLockingMultiplePrivateOwnedTest added to OptimisticLockingTestModel)
