When I have tomcat running in debugging mode and I save a class, I always get
the 'Hot Code Replace Failed' dialog pop up and then tomcat reloads the context.

I'm using tomcat 6 with WTP 'use[ing] tomcat installation' radio button selected.

What configuration do I require for Tomcat to allow hot code replace to succeed?

I'm using Eclipse 3.3.0 and WTP 2.
