I'm trying to wrap my head around the semantics of extension id's, and I have some questions that I haven't been able to find in the documentation.

1. Why are some extension id's #REQUIRED, while others are #IMPLIED? And how are #IMPLIED ones generated, anyway?
2. What does "fully-qualified" mean in the context of a extension id? And if an id is not "fully-qualified", then I understand it gets the plugin's id prefixed to it -- is this correct? If so, where else does this apply?
