I have the Eclipse Forms Examples proejct in my workspace, checked out
from CVS (module org.eclipse.ui.forms.examples). It contains the class
org.eclipse.ui.forms.examples.internal.dialogs.SampleFormDia log which
extends org.eclipse.ui.forms.FormDialog
