What is the difference between ContinuityOfCareDocument2 and ContinuityOfCareDocument? I see this distinction for other elements in the MDHT model. Is there documentation that would tell me this?

So if I want to conform to the 2.1 spec I should use the xxx2 versions of model objects, including the factories I suppose?
