I would like to extend team/commit such that the menu option displays a
customized dialog box. The new dialog would add information to the commit
comment to provide a site-standard format for commit comments.

Is this possible? If yes, which extension point should I be looking at?

There is currently no way for third parties to participate in the CVS
commit dialog. I do recall seeing requests for similar functionality in
Bugzilla so you may want to search the Platform/CVS and Platform/Team
component bugs to look for related requests. However, the feature you
describe sounds like a generally useful feature so it may be reasonable
to make a specific request for the desired functionality instead of
making a general request for extendibility (i.e. getting extendibility
right is very complicated). There was already a contribution that added
client side commit templates. Perhaps you would consider providing a
contribution that extends that behavior to meet your needs?
