Perhaps not individual items (without more information), but you can
programmatically add and remove action sets:

IWorkbenchPage#showActionSet(*)

I've added a toolbar to my coolbar.When i switch
the perspective the toolbar should appear or disappear.

Use the org.eclipse.ui.actionSets and org.eclipse.ui.perspectiveExtensions
extension points. Please read the documentation.
