Those books must be referring to the default plug-in structure
that is chosen when you do not select the check box to
generate a plug-in from a template (like the Hello World plugin).

In other words, if you don't select a template for a custom plugin,
you get one with the default plugin structure.

For anyone else who may be creating a plug-in using the Plug-In
Development Environment (PDE) and using a 2.x book (ie, "the Java
Developer's Guide To Eclipse", The O'Reilly books (including those that
claim to cover 3.0) for instruction:

The "Hello World" plugin "from scratch" examples describe a
dialog titled "New Plug-in Project" and a heading of "Plug-in Code
Generators". You are instructed to select "Create a plug-in project
using a code generation wizard", and then from the enabled list box
select "Default Plug-In Structure". This appears to be analogous to NOT
selecting "Create a plug-in using one of the templates" in the dialog by
the same name (but a heading of "Templates") - instead just use the
finish button. This will result in the generation of a (skeleton)
plug-in manifest (plugin.xml) and a plug-in top level Java class.
