Is there anything similar to the 3.x workbench DecoratorManager in e4, or do I need to roll my own?

I also have the problem that I cannot use the ILabelDecorator, which is provided by the DecoratorManager. As the DecoratorManager uses PlatformUI in its constructor, there is no way to use it.

It seems that this feature shouldn't be used any more, as there is the possibility to define the styling with a CSS-file. With CSS it should be possible the specially define stylings.

The disadvantage of this way is that decorations are not possible any more, especially for images. I had to copy and adapt some images to get the decoration effect back, but it is only a replacment of images, not a decoration.

I don't think you can replace that easily with CSS - I'd rather say that
someone should take a the code and make it available from a bundle that
does not require the compat layer.
