In PDT 2.1 it looks like information was not being correct saved to hard
disk in regard to breakpoints. That meant that you can create a
breakpoint and it would work, but if you shutdown PDT and restart it
then the breakpoint would not work.

People also found a similar situation when initially migrating from PDT
2.1 to PDT 2.2

The solution is to delete all breakpoints and recreate them after
starting PDT 2.1 every time.

The problem looks like it is fixed in PDT 2.2 (ie breakpoint information
is persisted correctly), so when migrating from PDT 2.1 to PDT 2.2 you
just need to delete all breakpoints once.
