this might be the probelm. This was easy do it in the earlier release of
STEM, byt specifying a county in the US form the drop down menu. How this
is done in the new version?

What version are you running? (The 0.2.0M1 RCP version or newer code from
the repository)?
Are there any messages in the Error Log view?
When you start the simulation does it appear in the "Active Simulations"
view?
Does it appear in the "Control View"?
If you select your simulation in the Active Simulations view does the map
draw?
Does the model properties section of the Map view get populated with the
decorators and their properties?
Does the progress monitor in the bottom right indicate that the simulation
is running?

I suspect the model isn't quite "right", which is not your fault, the
documentation is lagging to explain all the details. But, I suspect your
model is not structured "properly". Create a bunch of intermediate models
for each thing you add, e.g., create a model "tree" with the Earth (ZZZ)
Model and Edge graphs as sub-components and have it include another model
that includes the USA (0,1,2) Full infrastructure. Create another model
above this that includes the disease decorator. Save that model and then
include it in the Scenario along with the Sequencer and Infector.

Our apologies about the documentation, it's coming.

Is STEM (maybe also other OHF project, but here it seems obvious) also
considering usage of the Eclipse RAP project for things like Google
Maps, etc.?

It seems this could allow a RCP only approach for mapping where the
current map view in STEM seems to rely on the entire Google Earth client
installed (I checked it for my article, but for the conference demo in
Amiens I used only the Offline option without Google ;-)

P.s.: I am not sure if I have already shared this event with you in
regards to time/date and probably also my slides used there. As far as I
remember, my Eclipse Magazin article on OHF was mentioned in the Wiki,
but I don't think the STEM one or the Libre Software Meeting did...

Please let me know, if you need information on those?

Using RAP for STEM visualizatios makes some sense and I doubt it would
require any specific changes to STEM. We've also implemetned (but not
checked in yet) a visualization interface based on the NASA World Wind
http://www.worldwindcentral.com project.
