Replace tabs with spaces seems to be cached somehow. I used to want them replaced, but now I don't want them replaced anymore. I've changed the settings and checked them several times but Eclipse still replaces tabs with spaces when I hit the tab-key in old files. In new files tabs are not replaced with spaces just like they shouldn't be.

How do I clear out the cache so it doesn't replace tabs with spaces even for old files?

Check your projects' properties -- perhaps you have some projects with per-project settings? Project preferences override the global preferences.
