The TOC is built in the designer and will automatically be seen in a pdf
that is generated in pdf. Is this not what you are looking for?

No, that builds bookmarks that are shown on the left in acrobat reader.
Those are not part of the document itself (e.g. when printed). What I was
looking for was ability to have those same items as a book style TOC in
the beginning of the pdf, i.e. an enumeration of TOC entries w/
corresponding page numbers.

I am not sure how to do this, but you should be able to add the itext
jars within the itext plugin to your build path and write code using
them in an application that uses the REAPI.
