Under what conditions does Source->Format mark a file as having been
modified? I'm pretty sure that I've seen it mark a file even when it made no
changes to the file's text, but I've also seen this command not mark a file
as having been modified. Is there a general rule? Thanks.

I bet there were some whitespace changes that you did not see.

The document is marked modified if it changed. If you see a case where
this is not true then please file a bug report.
