It looks like CDT supports simple refactoring (renaming) as of 2.0, based
on the docs that I've looked at, but the option doesn't appear when I try
to use it via the refactoring context menu. Instead I get "<no
refactoring available>". BTW, I'm using Eclipse 3.0.1 and CDT 2.1.0. Any
ideas?

I'm having the same problem but I have no idea why. I'm using Eclipse
3.0.0 with "C/C++ Development Tools" add-in version 2.0.2 on RedHat
Linux. What're you using?

The feature is significantly limited.
Different contributors are looking into upgrading this funcitonality for our
next release.

BTW, I'm on WinXP. Based on help docs, it looks like I _should_ be able
to right-click a class name in a C++ project header file and do a
refactor-rename from there. Should I be able to do this, or is this not
available yet either? Also, what is the approximate next release date?

Try doing it from the outline view context menu instead of from the editor.
