versions of eclipse (3.2.1) and wtp (I believe 1.1.1 ?). Perhaps some
of the information is out of date?
I think my problem is that I have no idea how to deploy and run the
application from within eclipse.
Can someone point me to a recent tutorial that works?

Aha! I did not realize you have to select the servelt first, and then
attempt "run as." That worked much better.
