For rotating the text label, take a look at this plug-in from the BIRT Exchange Marketplace:

BIRT Exchange Marketplace - BIRT Controls Lib

For the aggregation column in crosstab, I'm not sure I'm totally understanding the issue. If you're talking about the total field, when you set it up in the grand total editor, there's an option for position that can be changed to make the aggregation column first. Let me know if you're talking about something else.

The rotated text item creates an image, I believe. The standard XLS emitter does not support images. This is probably why it doesn't work for you in XLS. If you try another XLS emitter, like this one: BIRT Exchange Marketplace - Spud Soft BIRT Excel Emitters, it might work for you, cause there is image support. You might give it a try. As far as the other outputs, it should at least work in pdf, but I don't see why the others wouldn't work, as well. The code for creating the rotated text element can be found in the BIRT source code. For 4.2.2, you might have to go get the source and build the plugin project yourself.
