I want to know how to initialize "Form" API Widget, i trying but i cant find how to do this.

Your screenshot shows that you're using a GUI builder of some sort
(Window Builder Pro perhaps?). You should look into whether there's a
newsgroup/forum for questions related to the tool you're using, because
this is not an issue with SWT itself.
