The option to right-click a file (for example a .php file) seems to come and go. Sometimes it is there, sometimes it is not, and I have to do right-click->refactor->rename

This is a new issue since I upgraded. Can someone help? I rename files a lot and this is getting pretty frustrating as I can't find a pattern as to why it works when it does, and doesn't work when it doesn't!
