When I have a grouped result set, and I create a TOC entry on each group I
get these really nicely nested TOC entries. However, often times I would
like my manually specified TOC entries to nest, but I can't seem to make
that happen.

This is what I would like to have

Top Level TOC Entry
Nested TOC Entry
Nested Even More
Another Top Level Entry

This works great for grouped result sets, but not at all for manually
created TO entries.

Any suggestions?

This would be a great enhancement request. Can you log it in bugzilla.
You can do something like put empty tables nested within an existing
column to create the effect, but that is probably not the best thing for
performance.
