I created a program in Eclipse for a survey for my company, and being not to proficient in HTML, is it possible to migrate the code into a webpage? My end goal is to have a website that allows you to take a survey, using my java code.

There definitely is not. The tools can help make technical challenges easier to solve and implement, but you have significant and fundamental architectural changes to learn and consider before you even get to that stage.
