What does the icon below in Outline view (looks like '#') means? And why two same objects have different icons? Is it bug? I didn't find such icon's explanation here. Documentation's miss?

The outline is helping here, but the documentation has (as you correctly identified) let you down.

The outline has different images because the two things are different, the first is a definition, the second a declaration.

As for the documentation, fixes are welcome. Please file a bug report, or for a simple thing like this you could just fix the documentation and contribute a gerrit. The file in question is checked in here: http://git.eclipse.org/c/cdt/org.eclipse.cdt.git/tree/doc/org.eclipse.cdt.doc.user/reference/cdt_u_outline_view.htm
