additional refactoring to me would be Extract Method, and almost as
important would be Inline Method, Push Up, and Pull Down.

Nothing happens in open source without contributions. No one has found
these "essential" enough to contribute these, so, yes, they aren't there.

On the plus side, there is some activity happening around refactoring
but this is probably a while from coming together.
