http://code.google.com/a/eclipselabs.org/p/xtext-forms-integ ration/ comes to mind. It might not completely match your use case, but should be instructional enough.

Well, how about this: http://code.google.com/a/eclipselabs.org/p/xtext-forms-integ ration/source/browse/#svn/trunk/plugins/org.eclipselabs.xtfo .demo.metamodel%3Fstate%3Dclosed

Google Code has a layout which doesn't use huge buttons, but rather small links, so it takes a bit of getting used to. It does work quite well.
