CABA,

A probable cause for it, that you have run stand-alone system and search,
and later added more documents to your plug-ins. The new files were not
detected by search. Documents are indexed on the first launch of search
after installing plug-ins or upgrading (the version of plug-in changes).
The index is stored in the configuration area
eclipse\configuration\org.eclipse.help.base\index. Try deleting the index
directory, start stand-alone help and test if it works then. When upgrading
documentation later on in a plug-in, ensure the plug-in version is
incremented.
