Some menu items are not displayed in E4 application [message #894196]

I played around with Eclipse 4.2 RCP to figure out how to wire the view layer with the control layer. However, after a while I noticed that the "Help" menu item isn't displayed anymore, although the "To be rendered" and "Visible" checkboxes are both checked. After that I started a new project and added a Handled Tool Item, but the item isn't displayed. What might have gone wrong here?

Are you using org.eclipse.ui (the 4.2 Workbench) or an Eclipse4 application (many org.eclipse.e4.* bundles)?

Is there anything in your error log? <workbench>/.metadata/.log
