In which case the same class name appears in different lines?

I think a screenshot will be the much clearer :

You may have classes with the same name loaded with different classloaders, i.e. more than once. For every such class there is a separate row in the histogram.
When we build the histogram objects are grouped by the ID of the class and not by the name.
